Need help with a little mtb diagnosis !help!

Hi, this is my first post and am coming to you guys for some help.
The problem is that my chain skips when i apply a lot of pressure on my pedals (like when going up a hill) this results in the pedals jumping a few inch and not having a very smooth ride, also sometime the chain slips off totally and jams in between the cassette and supporting arm resulting in myself being thrown over the handle bars which aint too fun after the first time. My bike isnt too old and hasnt seen much wear and tear so if you could help my identify the problem and give any advice i would be extremely grateful.

Something sounds a bit worn - like the chain or someat. If it were me I’d be taking it down for a service. If you want to ride a bike you do have to keep them in fine fettle not just chuck em when they don’t behave. Imagine if we just chucked our wives/husbands away when they didn’t behave! Hmmm, now there’s a thought....
